This form includes a form for any minor (who is of driving age) to be able to receive care without the presence of a parent or legal guardian.

This form also includes a section on allowing other people to bring your minor to receive care, such as a family member or friend.

NOTE: The parent or legal guardian MUST be present for the initial visit.
