To shorten your initial office visit time, please fill out our digital paperwork. You will receive a PIN from the office when you schedule the appointment in order to access paperwork.
For accident cases, please indicate in the paperwork that it is accident-related.

Minor New Patient Forms
Please indicate to the office when scheduling the appointment is for a minor so we can digitally send appropriate forms.

This form includes a form for any minor (who is of driving age) to be able to receive care without the presence of a parent or legal guardian.
This form also includes a section on allowing other people to bring your minor to receive care, such as a family member or friend.
NOTE: The parent or legal guardian MUST be present for the initial visit.
